At Strategic Wound Care, we promote fast and effective wound healing by following our patients through every level of their care: hospitals, long-term acute care facilities, skilled nursing facilities, nursing homes, and home health agencies.

Strategic Wound Care has contracted with specific healthcare facilities (Skilled Nursing Facilities, Long-Term Acute Care facilities, Home Health Agencies, Wound Care Clinics, and Acute Care Hospitals) to provide services. The goal is to establish Strategic Wound Care as the expert in wound care for each facility, thereby giving Strategic Wound Care the ability to consult, as well as the ability to create open lines of communication and education for a standardization of care for the health personnel working in the facility. Because Strategic Wound Care operates on a pod concept of establishing relationships along the medical continuum, Strategic Wound Care can provide continuity of care for patients as they progress through the healthcare system.

In addition, this continuity helps to develop cooperative efforts among the health care facilities, as they are able to increase communication with each other due to the cooperative objectives of meeting the health care needs of any specific patient. The result has largely been exceptional on many levels. Most significantly, patient outcome has improved with continuity of care. Cooperative services, even from competing agencies, have improved.
